A Case of Cochlear Implantation Inserted into Internal Auditory Canal in Severe Malformed Cochlea

Abstract
Cochlear implantation is a beneficial method of rehabilitation in deaf children. However, some congenital deaf children may have anatomical malformations of the inner ear. This case study is to report postimplantation performance in patient with Mondini inner ear malformation with entrance of electrode into internal auditory canal (IAC). The patient underwent implantation with Nucleus 24-channel cochlear implant at the age of 8 years. Radiological assessments using CT and MRI revealed bilateral Mondini deformities. 18 electrodes were inserted, and thirteen electrodes were active. Postoperative X-Ray demonstrated the entrance of electrode into IAC. During postoperative mapping, IAC electrodes were excluded. He showed limited sound response and could not perform the formal speech perception tests for initial several months. After training, the abilities of speech discrimination and spoken language were improved in comparison with those before operation. His open-set word recognition ability was 6.67%. However, it was 30% at 6 months following implantation and his sentence comprehension rate is 70% at the 2 years following implantation. This report demonstrates that cochlear implantation can be a successful method of rehabilitation in patient with congenital deafness who have Mondini inner ear malformation.
